The new guidelines that came out in 2016 illustrated the most critical determinant of risk equals current excessive daily dose by actual weight in the following manner Uunder 5 mg/kg=2 percent risk over 10 years with an increase sharply to 20 percent at 20 years. 4 Furthermore, at 800 mg dosing, the daily risk would be 25 to 40 percent in one. A risk factor for Plaquenil hydroxychloroquine retinotoxicity is a daily dose that exceeds 5.0 mg of drug per kg of body weight. The tool on the right simply calculates this threshold based on a ppatient’s real body weight. It’s important to understand that the daily dose is only one risk factor for plaquenil retinotoxicity. Aug 29, 2014 Several risk factors may increase the likelihood of retinal toxicity from Plaquenil such as, age of greater than 60 years, daily dose more than 6.5 mg/kg; use of the drug more than 5 years, obesity, preexisting retinal disease and, renal or liver failure.

It is imperative that patients and physicians are aware of and watch for this drug’s ocular side effects. Retinal toxicity from hydroxychloroquine is rare, but even if the medication is discontinued, vision loss may be irreversible and may continue to progress.

The risk factors reported by the American Academy of the Ophthalmology AAO were compared between patients with n = 5 and without toxicity n = 26 findings. Risk score was calculated for each patient. Risk factors for retinopathy normal daily doses range 75-300mg toxicity has been reported with doses of 1200 to 2400 mg/day for 1 to 2 years. Mechanism - Inhibition of retinal enzymes and phototoxicity have been suggested as possible causes of the retinal degeneration. Research has shown that your risk of developing retinopathy is higher the longer you are taking Plaquenil. In fact, the incidence of retinal toxicity rises sharply in people that have taken it between 5 and 7 years. As a result, baseline testing should occur within the first year of taking the drug and then annually at the five-year mark.

He went to see a Rheumi and he said it could be Fibro/Lupus or worst case... This has been so stressful for us as the wait for the bloodwork is the worst. The Rheumi put him on 75 mg of Lyrica to treat him for fibro in case that was it but it knocks my husband out so he can't take it. My sister was diagnosed with scleroderma at age 6 and is extremely ill now at 30 she can barely move-my mother was diagnosed with lupus about 4 years ago and my aunt also suffers from lupus.
