Since HCQ is an immunomodulatory drug has an inhibitory and an immunomodulatory effect on T cells and interleukin 1 and interleukin 6 and type 1 diabetes is an autoimmune disease, a possible reduction in islet-cell autoimmunity by HCQ could be the underlying mechanism since our patient has longstanding type 1 diabetes with no insulin reserve and an effect on insulin resistance is less likely to be the underlying mechanism that led to the improvement in glycemic control Ben-Zvi et al. 2011. Hydroxychloroquine, an alkylated 4-aminoquinoline, is a hydroxylated form of chloroquine and has a biological mechanism of action similar to chloroquine’s. Hydroxychloroquine was originally developed as a safer alternative to chloroquine for the treatment of malaria and was later found to have antirheumatic and anti-inflammatory uses. A study of about 200 patients with a history of hydroxychloroquine retinopathy, including 36 Asian patients, found that the pericentral pattern occurred in half the Asian patients but only 2% of the white patients. 31 The mechanism for this finding is unclear, but because pericentral retinopathy spares the macula, it can be missed using standard screening methods.

The most serious adverse effects affect the eye, with dose-related retinopathy as a concern even after hydroxychloroquine use is discontinued. The most common adverse effects are a mild nausea and occasional stomach cramps with mild diarrhea.

LymeMD Plaquenil alone? Dec 16, 2008 The regimen was changed to Amoxil and Minocin. The knee pain and other symptoms rapidly improved over the next two weeks. Then one week later he complained of confusion and unsteadiness. I diagnosed a "brain Herx." I added Plaquenil back, as well as Welchol, presumptively to remove "neurotoxins." I also reduced the dose of Amoxicillin and Minocin.
