In 2011 the American Academy of Ophthalmology published new clinical guidelines for appropriate annual screenings for those patients taking Plaquenil. These guidelines are much different from the previous 2002 recommendations, and many practicing doctors were trained with the 2002 guidelines in mind. Remember, any patient who is at a high risk for Plaquenil toxicity should be examined on an annual basis following the baseline exam. The goal of monitoring/screening is to identify early macular damage prior to irreversible vision loss or even before visible signs of bull’s eye maculopathy. Both my rheumatologist and opthamologist recommended exams every 6mo for the first year I was on plaquenil. Now I just go yearly to have my eyes checked. Also, my rhuemy said any time he makes an adjustment to my plaquenil that I'll need an eye exam within 6mo just to make sure nothing has changed. Good luck!

Plaquenil has long been associated with retinal toxicity which can lead to vision loss. In particular it affects the macular region of the retina which is responsible for central vision.

Detection of Early Hydroxychloroquine Retinal Toxicity. Multifocal ERG ring ratios reveal retinal toxicity in a surprisingly large fraction of patients taking hydroxychloroquine who are referred for evaluation and have no previous history of eye disease. We find the use of ring ratios to be a sensitive measure of dysfunction in ocular hydroxychloroquine toxicity.
